HOW TO ENTER :
To enter go to a participating Fast Tax $10,000 Give Away location and enter the sweepstakes by printing your name, address, daytime and evening phone number on an entry form and put it in the entry box at any participating Fast Tax location. You can also register on our website at www.fasttaxservice.com. The forms can be picked up at any participating Fast Tax location. Fast Tax Service, Inc. is not responsible for late, lost, postage due, or misdirected mail.
PRIZE :
Only one winner in this $10,000 Give Away. The winner will receive $10,000 cash. Odds of winning depend upon the number of eligible entries received.
JUDGING :
Winner will be selected in a drawing from all qualified entries received by April 1, 2008. The drawing will take place by April 21, 2008 at 11:30 a.m. at 4111 Jefferson Hwy, New Orleans, LA 70121. The drawing will be conducted under the supervision of Bob Mora, Certified Public Accountant, who reserves the exclusive right to interpret all conditions regarding this promotion free from claim for damages or recourse of any kind. By participation in this sweepstakes, entrants agree to be bound by the rules and the decisions of the judges, which shall be final and binding.
NOTIFICATION TO WINNER :
Winner will be notified by phone and/or mail and will be required to sign an Affidavit of Eligibility and Talent Release Form, which must be returned within 14 days. If the affidavit is not returned within this time period or is returned from the Post Office as undeliverable, an alternate winner will be selected. Winner grants permission for the use of his/her name, photograph, video or likeness for advertising and promotion purposes without additional compensation or permission. Winner can not substitute or transfer prize. All taxes imposed by any taxing authority on account of this give away are the sole responsibility of the winner. In the event of a jointly filed tax return, a single prize will be awarded to the couple filing the tax return. Winner must show proof of ID.
ELIGIBILITY :
Contest is open to residents of the United States 18 years of age or older. Employees of Fast Tax or any employment agency doing business with Fast Tax and their respective immediate families (husband, wife and children), and all other agencies and persons connected with conducting this “Give Away” are not eligible. All federal, state and local laws apply. Void wherever prohibited or restricted by law.
